ASSESSMENT BOOKING FORM (general) – Level 3 Award in Energy Awareness 6281-01

This form is to register for your Level 3 Award in Energy Awareness assessment. These details will be used to register your assessment with the awarding body.

Before submitting this form, learners should already be booked onto the Energy Awareness course (face-to-face or e-learning) and have been given an exam date. This form is to confirm the exam booking.

SPECIAL LEARNING REQUIREMENTS

Exam Access Arrangements (EAAs) are the reasonable adjustments that can be made for a candidate to access an assessment. This might include things like extra time to complete an exam paper, permission to use assistive technology, provision of rest breaks and so on.

Access arrangements can be applied in certain circumstances such as for learners with special educational needs, disabilities, temporary injuries/illnesses and so on.

CANCELLATIONS

As outlined in the main course booking, the following cancellation periods / charges will apply should the learner wish to cancel their chosen exam date once booked:

> Less than one weeks notice 75% of the fee will be payable
> Less than three weeks notice 50% of the fee will be payable
> Over three weeks notice will not incur a charge
> If you received a fully funded place please inform NEA of any cancellations as soon as possible so that your place can be re-allocated.

Cancellations should be made in writing.
